Is my Data Secure?
Yes. Our form entries and data is stored securely and encrypted for your security.
Do I need to have a business associate agreement (BAA) in place for HIPAA?
No. We do not store PHI in our system and for this reason we do not require BAA’s. We rely on the client/customer to ensure that PHI is not entered into the form. We provide all clients with instructions on how exam identifiers should be handled and how to avoid HIPAA security concerns.
How do I subscribe to both accreditation categories?
To subscribe to both categories, you will need to register for each category subscription separately. You can use the same email address and use a separate password for each subscription account. You will need to login to each subscription separately.
We passed accreditation, should I continue my subscription?
Once you’re accredited, it is important to continue performing the Quality Improvement reviews on a regular basis. Your subscription will continue until cancelled by you. Our Accreditation Quality Improvement options within your account will be available for your accreditation requirements throughout the term of your accreditation and will ensure that you are ready when you are audited by the IAC.
How do I access my form/account data?
Your form data is available to you at the password secured link which was emailed to you at the start of your account set up. If you need to have that email resent to you, please send an email request to firstname.lastname@example.org and reference your account ID number.
How long is my subscription for?
Your subscription is an ongoing subscription and will renew monthly until it is cancelled by you.
What happens to my data if I cancel my account?
You always have the option to store your data locally and to download it from your account, while your account is active. Account data is not stored for inactive accounts. Once cancelled, your account access will be terminated immediately and your submitted data will be deleted.
How do I cancel my account?
Please send an email to email@example.com with the subject “Cancel Account”. Please list your account number within the cancellation request email. Please note that account cancellation requests sent via email require 30 days notice to process. Once cancelled, your account will be terminated immediately.
